  • Transfer Improvement Action Collaborative Application

  • Thank you for your interest in our 2025 Step Up Together Community Birth Transfer Improvement Action Collaborative. This program is designed specifically for interdisciplinary teams from both community and hospital settings who are jointly invested in improving their collaboration, emergency preparedness, and transfer process. This year's Action Collaborative will have 2 cohorts: 

    • A birth center cohort, for teams preparing to run a Full Transfer Drill scenario originating in a freestanding birth center.
    • A home birth cohort, for teams preparing to run a Full Transfer Drill scenario originating in a home birth.

    Thanks to generous funding from the Skyline Foundation and a partnership between Primary Maternity Care and the National Association of Certified Professional Midwives, funding is available for 12 teams in each cohort to participate at no cost. Each team is comprised of representatives from:

    • community birth practice (e.g. lead midwife and/or lead birth assistant)
    • hospital unit (e.g. nurse manager or education coordinator)
    • EMS or transport partner (e.g. education coordiator or medical director)

    These interdisciplinary team leads will participate in the webinars, receive program communications, and work together to plan and execute the drill in their community.

    We ask that one individual from either the community birth practice or the hospital complete this application. The application should include a letter of intent from the other partner and, if available, from the EMS or transport partner.

  • All participants in the Action Collaborative are expected to:

    • Attend 7 live, 1-1.5 hour sessions or view recordings (between September 22 and January 21)
    • Review background materials provided before program launch and prior to each session
    • Work together to use the tools and coaching from the program to plan, conduct, and debrief:
      • Partial Transfer Drills in the community setting and in the hospital setting
      • a Full Transfer Drill that begins in the community, moves by emergency transport, and resolves in the hospital setting.
